This form is required to change the status of machine maintenance inlcuding a machine from a Status 33 (installed not on maintenance) to a Statis 4X (out of productive use).

Contract Change Request Form (.doc) Form
The IGS Contract Change Request Form, found on this page, is used to obtain approvals to modify terms on a standard agreement.

Use this form for equipment modifications. Must have Excel 2002 or later installed on your computer to complete this form.

The information submitted in this form will be used by MSS Deployment Team to initiate newly ordered Managed Security Services. Submit the completed form as part of the order package.

This form is used to request a renewal contract quote for IBM ISS Services. Form must be completed and sent to issrenew@us.ibm and the Service Contract Sales Representative.
InfoPrint Solutions Contract Change Request Form (.doc) PDFForm (89KB)
This form is required for Contract Change Requests for InfoPrint Solutions Company (IPS) agreement clarification. Contract Change Request have to be approved prior to submission for contract entitlement. All appprovals should be submitted with contract packages to IBM Business Customer Support Operations (BPCSO).

This form is only to be used for signed contracts which have not yet been entitled by Business Partner Customer Service Organization. Early entitlement is valid for 30 days.

ISAT Inventory Form Web page
This link takes you to right to the form required to update end-user customer inventory associated with a maintenance contract not yet accepted by IBM (RFA'd).
Maintenance Contract Status Request Form (xls) Form (33KB)
Used to request Maintenance and Remote Technical Support Service status of installed equipment outside of a Business Partner enterprise with end-user authorization. Also used for CHIS/INV Pull requests.
Maintenance Quote Request Form (.doc) Form (57KB)
Used to request a maintenance quote for IBM hardware maintenance that is not configurable on the IBM Services Assistant Tool.
Non-IBM Product Support Qualification Forms
These forms are used to qualify the non-IBM inventory for each of the respective vendors. This inventory qualification must be completed before proposing or contracting for support from IBM.
Remote Technical Services (RTS) Special Request Form Form (105KB)
Form is used to request special pricing consideration for proposals that include Remote Technical Support Service offerings that are not available in IBM proposal/ordering tools. This may include competitive opportunities that are above the 2500 machine count limit for Support Line - OS, MSApps and Vmware, Operating System Extension Support, After License Fees, Premium Support Services - Account Advocate and Enhanced Technical Support opportunities. This form should also be used for SWMA renewal pricing when the quantity of machines exceeds a minium number and the required Total Contract Value (TCV) threshold has been met.
Retro Request Approval Form Form (36KB)
Use this form to get approval from IBM Global Services when contracts are sent to Business Partner Support Operations (BPSO) for processing after the contract start date. The contract start date is the date indicated on the Statement of Work (SOW) or the customer signature date, whichever is later. Send request to BPASSIST with the following information: Retro Request Form (fully completed), all supporting documentation, signed SOW, and ISAT solution summary.
Retro Cancellation Request Form (.doc) Form (37KB)
A one page form you can use when requesting retroactive cancellation.
Retro Request Extension Approval Form Form (36KB)
Use this form to get approval from IBM Global Services when contracts are sent to Business Partner Support Operations (BPSO) for processing after the contract start date. The contract start date is the date indicated on the Statement of Work (SOW) or the customer signature date, whichever is later. Send request to BPASSIST with the following information: Retro Request Form (fully completed), all supporting documentation, signed SOW, and ISAT solution summary.
Request to Transfer Services Contract Form PDFForm (6KB)
This form must be submitted to Business Partner Support Organization (BPSO) when maintenance or Support Line service is transferred (removed) from an IBM contract to a remarketed Business Partner Contract or from one Business Partner contract to another. End user signature is required. Termination penalties may apply

Software Maintenance After License Fee Request Approval Form (66KB)
This form is required when the Software Maintenance (SWMA) After License Fee (ALF) is applied as a result of a lapse in Software Maintenance coverage. Form must be sent to BPASSIST for approval. Upon approval, the SWMA ALF Approval Form should be sent with the Services contract package to Business Partner Customer Support Operations (BPCSO).
Upgrade Fee Request Form Form (36KB)
Use this form to get approval from IBM Global Services when contracts are sent to Business Partner Support Operations (BPSO) for processing after the contract start date.
